To encourage our patients' cooperation in achieving treatment goals, we offer a fun incentive program. Here is how you earn tokens to trade for a variety of prizes.

Patients with Braces

  • Arrive at scheduled appointment time
  • Keep your mouth, teeth, and braces absolutely clean
  • Have no loose or broken bands/bonds
  • Wear your Campbell T-shirt to your appointments
  • Have your Cleaning Reward Card signed by your dentist/hygienist at your regular cleaning

Patients with Removable Appliances

  • Arrive at scheduled appointment time
  • Keep mouth, teeth, and appliances absolutely clean
  • Wear your appliances as instructed and bring "time worn" record book to appointment
  • Wear your Campbell T-shirt to your appointments
  • Have your Cleaning Reward Card signed by your dentist/hygienist at your regular cleaning.

Each quarter Dr. Campbell and Team choose a charity to support. Many of our patients donate their tokens and Dr. Campbell pays 50 cents for each one donated. Here are recent charities that have received financial support from Dr. Campbell and his patients.
